Terms and Conditions
Welcome to Face and Body Membership!
Please note all the below terms and conditions regarding all of our Memberships:
- Limited number of memberships are available.
- A one-time enrollment fee of $25 will be charged for all new Members.
- All monthly treatments have to be redeemed in the same month (between the first and the last day of the month).
- Unused membership treatments will rollover and can be used within 12 months – as long as you keep your Membership current.
- Appointments must be canceled 24 hours in advance. Failure to timely cancel or show up will result in a full usage of your monthly treatment.
- Memberships and benefits are non-transferable and non-refundable.
- Memberships can’t be combined with any other offers.
- Membership fee is billed on the 1st day of each month. Your credit card and email address are required to participate in our Membership Program. If we can’t process your membership payment by the 5th day of the month, your membership will be frozen and you will incur a $35 late fee. You will have until the last day of the month to pay your membership fee, along with a late fee without losing your treatment that month. If the account is still delinquent by the 1st day of the following month, your membership will be cancelled.
- If you do not make payments for the minimum contract period of 6 months and cancel for any reason during this time, you as the buyer are responsible for a $149 early termination fee or the total amount of payments due under the contract, whichever is greater.
- Memberships are a minimum of six (6) months.
- Membership prices do not include any applicable taxes. Facial and body treatments will include 4% NYC Sales Tax at time of service.
- Opulence Glamour Skin Clinic reserves the right to modify the terms and conditions of all memberships upon advance prior written notice.
- By signing your Membership Agreement, you, as a Buyer acknowledge that Buyer is of legal age; has received a filled-in and completed copy of the this Membership Agreement that identifies the Membership type; has read and understands the entire Membership Agreement that includes but not limited to the CC Request (if applicable); and has an understanding of the Release & Wavier or Liability & Indemnity.
- There are no strings attached. We are confident that you will love your membership, however, should you decide to discontinue, simply provide us a 30-day written notice after your first six (6) months of membership and your membership will be stopped. Cancellation requests should be sent via email to: info@OpulenceGlamour.com.
- As of the effective date of a cancellation, any unused rollovers expire.
- Any discounted membership gift cards may not be used by a member any time during or after the cancellation of membership.
- Memberships may be placed on hold for a fee of $25/month. Requests should be emailed to: info@opulenceglamour.com at least 15 days in advance (always by the 1st day of the month).
